Age Requirement for Completing Form: 18+

I am, or my legal guardian is, aged 18 years or over and can legally sign this agreement.

Inherent Risks of Rock Climbing

I understand that indoor rock climbing involves risks that may cause various injuries, including death or serious disability. I acknowledge that indoor climbing is physically demanding, and participation in such activities may cause injury or bodily harm to myself.

Release and Indemnity from Compensation

I hereby release and indemnify World Gym Bundaberg, its subsidiaries and associated bodies, its servants and agents, the owner of the premises, and any other person involved in my participation in indoor climbing at World Gym Bundaberg from all actions or claims for compensation arising from my participation, including personal injury, death, or damage to property.

Ongoing Validity of Waiver

I agree that this waiver is ongoing and will apply to all future occasions on which I participate in indoor climbing at World Gym Bundaberg.

Contractual Document

I acknowledge that this document is contractual and may be relied upon in any proceedings by me, my heirs, executors, and assigns. This waiver, release, and indemnity is given in consideration of World Gym Bundaberg permitting me to participate in indoor climbing.

Compliance with Instructions

I agree to comply with all instructions given to me by World Gym Bundaberg relating to my participation in indoor climbing. I agree to indemnify World Gym Bundaberg, its servants and agents, and the owner of the premises against all liability that may incur, including legal costs, caused by my negligence or failure to comply with such instructions. I will not conduct activities within World Gym Bundaberg for which I am not certified.

Medical Conditions

I warrant that I do not suffer from any medical conditions that may affect my ability to participate safely in activities at World Gym Bundaberg.

Image Release

We’re proud of our venues and those who choose to take part in climbing. To promote the sport and World Gym Bundaberg, we publish visual media taken at our venues in various formats, including online and non-social media. Recordings do not aim to capture individual members unless directly requested. However, you may be visible and identifiable in the background. I consent to World Gym Bundaberg using images in which I appear for promotional purposes. I waive any rights and claims to any benefits in connection with the use of these images.

Participant Age Requirements:

• Minimum Age:Children must be 7 years old or older to use the rock-climbing wall.
• Supervision:A parent or guardian must supervise children during the entire visit.
• Induction:Participants will receive training from our staff on using the rock-climbing wall and equipment.
• Responsibility:After induction, parents/guardians are responsible for the safety of their child/children.
• Equipment Restrictions:Children under 12 years old may only use the rock-climbing wall; other gym equipment is prohibited for this age group.

By consenting to this waiver, you acknowledge that:

• Your child/children are 7 years old or older.
• You, as the parent/guardian, are responsible for their safety.
• Children under 12 may not use any equipment other than the rock-climbing wall.
